Forms All forms are available in both PDF and DOC format. PDF format can be printed using Adobe Acrobate reader. DOC files can be read and printed with Microsoft Word or Star Office word processors. The following 3 forms should be completed by all new patients: patient info sheet, patient authorization form and patient history form or infant form.
